3 Minute Introduction Script
- This is your Pregnancy Passport - it has information about
your pregnancy and the care you have received. It is for you to
keep and carry with you.
- Bring it with you to all of your appointments and carry it with
you all time in your purse or backpack so that you will have your
records with you in case of an emergency.
- Please read the Welcome card and the information in your Passport
before your next visit.
- We will fill out/ have filled out your prenatal test results
for you.
- Let us know if you have any questions.
- Tell us about anything that doesn’t seem right or problems
you are having immediately by calling _________ [phone]. We will
tell you what to do.
- Remember to bring it with you to all of your appointments and
give it to us when you check in.
